The short answer
Basic safety means freedom from unacceptable risk caused by physical hazards — electric shock, fire, mechanical, thermal and radiation hazards — under normal conditions and under a single-fault condition. Essential performance is the clinical performance whose loss or degradation would result in unacceptable risk (e.g. an infusion pump delivering the correct dose). The manufacturer defines the scope of essential performance — and this is exactly where audit gaps often appear.
Basic safety
Basic safety addresses the device's direct physical hazards. The central design concept is the single-fault condition: the device must stay safe even if a single means of protection or component fails.
- Electrical hazards — leakage currents, insulation, creepage and clearance distances
- Mechanical hazards — moving parts, stability, strength
- Thermal hazards — surface temperatures, overheating
- Radiation hazards — where relevant to the device type
Essential performance
Essential performance is not "everything the device can do" — only the performance whose loss creates unacceptable risk. Examples: the correct dosing of a pump, the reliable discharge of a defibrillator, the accuracy of a vital-sign alarm.

The link to risk management
Both concepts are tied directly to ISO 14971. What counts as "unacceptable" comes from your risk management process. Without a current ISO 14971 risk management file, the safety and performance requirements of 60601-1 cannot be demonstrated.
